It is of utmost importance that all clients of Revive Physio Care Clinic in Belleville understand that their privacy is taken into high consideration and is protected.  Under the Personal Health Information Protection Act (PHIPA) and the Health Information Protection Act (HIPA) in Ontario; all personal health medical documents related to treatment of any and all clients is protected and will not be shared to anyone outside of treatment and/or legal involvement; as necessary.

At Revive Physio Care, we want to ensure the safety and privacy of all our clients and want you to feel safe about sharing your personal information with us for the sole purpose of treatment.  Any personal and health related information you share will not be released or shared to anyone that is not apart of Revive Physio Care, or anyone that is not involved in your treatment, or anyone that does not have a legal obligation to you. If and when your personal health related information is shared with anyone of the above-mentioned involvement, you will be notified of their involvement and the release of your information.

Revive Physio Care will not sell or use your personal health related information in any marketing campaigns or marketing surveys at any time.

As a client you can request to have your personal health related information given to you at any time and have the right to not share any information with Revive Physio Care that you do not wish to.  You have the right to stop treatment and request your information back at any given time.
